FABRINET CONFLICT MINERALS REPORT YEAR ENDED DECEMBER 31, 2015

FABRINET
CONFLICT MINERALS REPORT
YEAR ENDED DECEMBER 31, 2015

I. Introduction

We provide advanced optical packaging and precision optical, electro-mechanical and electronic manufacturing services to original equipment manufacturers (OEMs) of complex products, such as optical communication components, modules and sub-systems, industrial lasers and sensors. We offer a broad range of advanced optical and electro-mechanical capabilities across the entire manufacturing process, including process design and engineering, supply chain management, manufacturing, advanced packaging, integration, final assembly and test. Certain products that we manufacture for our customers contain materials or components that use tin, tantalum, tungsten and/or gold (“3TG”). Due to the depth of the supply chain, we are far removed from the sources of ore from which these metals are produced and the smelters/refiners that process those ores; the efforts undertaken to identify the country(ies) or origin of those ores reflect our circumstances and position in the supply chain. The amount of information available globally on the traceability and sourcing of these ores is limited at this time; this situation is not unique to us. We have taken steps we believe to be reasonable under the circumstances to identify the applicable smelters and refiners of such 3TG metals in our supply chain; however, we believe that the smelters and refiners of the 3TG contained in the products we manufacture for our customers are best situated to identify the sources and countries of origin of the 3TG.

We do not directly manufacture products but “contract to manufacture” branded and generic products containing 3TG on behalf of our customers. Our due diligence efforts were undertaken on the exclusive and/or privately-branded products sourced directly by, and manufactured for, us in 2015.

II. Reasonable Country of Origin Inquiry (RCOI)

Direct suppliers were asked to provide answers to the Electronic Industry Citizenship Coalition (EICC) / Global e-Sustainability Initiative (GeSI) Conflict Minerals Common Reporting Template reflecting Fabrinet’s position on the supply chain (the “EICC Template”). The EICC Template includes questions regarding a direct supplier’s conflict-free policy, its due diligence process, and information about its supply chain such as the names and locations of smelters and refiners and the origin of 3TG used by those facilities. The EICC Template is regarded as the most common reporting tool for conflict minerals content and sourcing information worldwide, developed by several of the world’s leading consumer electronics companies.

We reviewed questionnaires received for completeness and consistency of answers. Suppliers were required to provide corrections and clarifications where needed. Based on the RCOI, we do not have sufficient information from our direct suppliers to determine the source of the necessary 3TG contained in the products we manufacture for our customers.

We believe our RCOI process was reasonably designed and performed in good faith, but there are inherent limitations in the information provided to us by third parties, including the possibility of information being inaccurate, incomplete or falsified despite our efforts to validate and confirm the information.

III. Due Diligence Program

We designed our overall conflict minerals procedures based on, and in conformity with the fivestep framework of set forth in the Second Edition of the Organisation for Economic Co-operation and Development’s Due Diligence Guidance for Responsible Supply Chains of Minerals from Conflict-Affected and High-Risk Areas. We implemented appropriate elements of the five-step framework as were reasonably applied to our circumstances and position in the supply chain as a “downstream” company with little to no direct influence on smelters/refiners. The five step framework consists of:

(1) Establish strong company management systems

 We have adopted and published our Conflict Minerals Sourcing Policy, a copy of which is publicly available on our website at http://fabrinet.com/services/supply-chain
 We have created a conflict minerals task force led by our supply chain management organization to implement our Conflict Minerals Sourcing Policy.
 We have communicated our Conflict Minerals Sourcing Policy to direct suppliers.
 We have conducted awareness sessions concerning the commitments and requirements expected of our suppliers

(2) Identify and assess risks in the supply chain

 We identified direct suppliers that supply product or material that may contain conflict minerals.
 We conducted supplier surveys using the EICC Template
 We reviewed responses to the EICC Template for completeness and consistency of answers. Suppliers were required to provide corrections and clarifications where needed.
 We compared smelters and refiners identified by suppliers to the Conflict-Free Sourcing Initiative’s (“CFSI”) list of validated conflict free facilities for initial risk screening.

(3) Design and implement a strategy to respond to identified risks

 We reported to senior management on direct suppliers’ responses to our conflict minerals information requests, including updates on monitoring and tracking corrective action and risk mitigation efforts (where applicable).
 Using a third party service, we compared smelters and refiners identified by suppliers to the CFSI lists of validated conflict free and verified facilities and the U.S. Department of Commerce “Reporting Requirements Under Section 1502(d)(3)(C) of the Dodd-Frank Act World-Wide Conflict Mineral Processing Facilities” (http://www.ita.doc.gov/td/forestprod/DOC-ConflictMineralReport.pdf) and conducted our own supplemental research on smelters and refiners.  We developed a risk mitigation plan that allows for continued trade with a supplier during the supplier’s risk mitigation efforts.

(4) Carry out independent third-party audit of supply chain due diligence at identified points in the supply chain

 We do not perform or direct audits of 3TG smelters or refiners. We rely on audits conducted by third parties through our membership in the EICC supplier self-disclosures of certification status, and our policy and procurement practices that encourage suppliers to purchase materials from audited smelters and refiners.

(5) Report on supply chain due diligence

 We communicated our Conflict Minerals Report on our company website at http://fabrinet.com/services/supply-chain

IV. Product Determination

We received inconclusive data from our direct suppliers, and material source information is still developing. Therefore, we are unable to make a definitive determination about the source of 3TG in the products we manufacture for our customers. At the same time, we received no information from our direct suppliers indicating that the 3TG in our Covered Products (as defined below) directly or indirectly financed or benefitted armed groups in the Democratic Republic of the Congo and the adjoining countries of Angola, Burundi, the Central African Republic, the Republic of the Congo, Rwanda, South Sudan, Tanzania, Uganda and Zambia. Some suppliers disclosed to us that scrap/recycled sources of 3TG were identified in their supply chains and did not require due diligence.

V. Product Description

Our customer base includes companies in complex industries that require advanced precision manufacturing capabilities, such as optical communications, industrial lasers and sensors. The products that we manufacture for our OEM customers include: selective switching products; tunable transponders and transceivers; active optical cables; solid state, diode-pumped, gas and fiber lasers; and sensors (collectively, the “Covered Products”). We identified the following smelters and refiners that have achieved Conflict Free designation by the CFSI or an audit program with which CFSI has mutual recognition, are actively in the process of obtaining the designations, or for which we independently obtained country of origin information. We were unable to definitively link the identified smelters and refiners to only those products and materials in our supply chain; therefore, the following list likely contains more processing facilities than are actually in our supply chain or Covered Products.

Countries of origin identified:
Angola, Argentina, Armenia, Australia, Austria, Belarus, Belgium, Bolivia, Brazil, Burundi, Canada, Chile, China, DRC- Congo (Kinshasa), Egypt, Estonia, Ethiopia, France, Germany, Ghana, Greece, Guinea, Guyana, Hong Kong, India, Indonesia, Italy, Japan, Jersey, Kazakhstan, Kyrgyzstan, Laos, Malaysia, Mali, Mexico, Mongolia, Morocco, Mozambique, Myanmar, Namibia, Netherlands, Niger, Nigeria, Papua New Guinea, Peru, Philippines, Poland, Portugal, Romania, Russia, Rwanda, Saudi Arabia, Sierra Leone, Singapore, South Africa, South Korea, Spain, Suriname, Sweden, Switzerland, Taiwan, Tajikistan, Tanzania, Thailand, Turkey, United Arab Emirates, United Kingdom, United States, Uzbekistan, Vietnam, Zambia, Zimbabwe. Covered Countries were identified only for CFSI audited facilities.

* Through our RCOI, certain suppliers identified Al Kaloti Jewelers Factory Limited (“Al Kaloti”), Fidelity Printers and Refiners, and Sudan Gold Refinery as sources of gold in their supply chains. These refineries are not audited by the CFSI and public information indicates the possibility that these are high-risk facilities. Because the conflict minerals information from these suppliers was presented to Fabrinet as “company-level” responses rather than Fabrinet product-specific responses, it is uncertain if these gold refineries are actually in our supply chain. We plan to conduct additional research on whether these refineries are, in reality, present in our supply chain. If we determine that material sourced from these refineries is present in Fabrinet's supply chain, we will work with our suppliers and the CFSI to conduct third party audits of the facilities. Should the facilities decline to participate in an audit, we will evaluate our relationship with that supplier and explore alternatives. In addition, one tin smelter (PT Pelat Timah Nusantara Tbk) was reported by some suppliers in their supply chain; however, this smelter suspended operation prior to 2015 and tin originating from this smelter is not expected to reasonably be in our supply chain for 2015.

VI. Steps to Improve Due Diligence

We will continue to communicate our expectations and information requirements to our direct suppliers. We will also continue to monitor changes in circumstances that may impact the facts or our determination. Over time, we anticipate that the amount of information globally on the traceability and sourcing of these ores will increase and improve our knowledge. We will continue to make inquiries to our direct suppliers and undertake additional fact and risk assessments where potentially relevant changes in facts or circumstances are identified. New suppliers will be reviewed for conflict minerals conformance during initial business reviews. If we become aware of a supplier whose due diligence needs improvement, we intend to continue the trade relationship while that supplier improves its performance. We expect our suppliers to take similar measures with their suppliers to ensure alignment throughout the supply chain.

VII. Independent Private Sector Audit

Not required for calendar year 2015.

Telefonaktiebolaget LM Ericsson - Conflict Minerals Report for the year ended December 31, 2015

Telefonaktiebolaget LM Ericsson - Conflict Minerals Report for the year ended December 31, 2015

This Conflict Minerals Report of Telefonaktiebolaget LM Ericsson for the year ended December 31, 2015 is provided pursuant to Rule 13p-1 under the Securities Exchange Act of 1934, as amended (the “Rule”). The Rule was adopted by the U.S. Securities and Exchange Commission (the “SEC”) to implement disclosure and reporting requirements pursuant to Section 1502 of the Dodd-Frank Wall Street Reform and Consumer Protection Act of 2010. As permitted by the Rule and the SEC, this report has not been subject to an independent private sector audit.

1. Introduction

Ericsson is a global provider of telecommunications infrastructure related products and services, including communications networks, services and support solutions. This report has been prepared by the management of Telefonaktiebolaget LM Ericsson (herein referred to as “Ericsson”, the “Company”, “we”, “us”, or “our”). This report does not include the activities of entities that are not required to be consolidated in our financial statements. In accordance with the Rule, this report is available on our website: http://www.ericsson.com/thecompany/investors/financial-reports.

2. Supply chain description and Reasonable Country of Origin Inquiry

2.1 General information on Ericsson’s supply chain

The hardware provided by Ericsson largely consists of electronics. The supply chain is complex with multiple tiers of suppliers. For manufacturing, Ericsson purchases customized and standardized components from several global providers as well as from local and regional suppliers, either off-the-shelf or in accordance with Ericsson’s design or specifications. Certain types of components, such as power modules, are produced in-house. The production of electronic modules and sub-assemblies is mostly outsourced to manufacturing services companies. Final configuration of products is largely done in-house and on-demand. This consists of assembling and testing modules and integrating them into complete units. A number of Ericsson’s suppliers design and manufacture highly specialized and customized components.

2.2 Overview of Ericsson’s Conflict Minerals Program and Reasonable Country of Origin Inquiry 2015

Ericsson gathers materials information from our suppliers. Through this we know that many of our products contain tin, tantalum, tungsten and gold (“3TG”) that are necessary to the functionality of those products. As part of Ericsson’s commitment to responsible sourcing, human rights and sustainability, it is our goal that the 3TG in our products should not, directly or indirectly, finance or benefit armed groups in the Democratic Republic of Congo or adjoining countries (the “DRC”). At the same time, we aim to continue supporting responsible mineral sourcing in the region. Ericsson’s efforts with respect to 3TG are aligned to the work of the Conflict-Free Sourcing Initiative and our aim is to improve the traceability in our supply chain. Ericsson does not ban materials from the DRC or avoid using 3TGs. The Company has a program team to fulfill compliance with the Rule, perform the Reasonable Country of Origin Inquiry and report on 3TG. This team consists of representatives from across the organization having specialized expertise such as sustainability and environmental expertise, sourcing and legal affairs. This team is responsible for Ericsson’s 3TG implementation work and senior management is regularly informed about matters relating to 3TG.

Ericsson relies on its first tier suppliers to provide information on the origin of 3TG contained in components and parts included in Ericsson’s products. This includes sources of 3TG that are supplied to them from sub-suppliers. Ericsson’s first tier suppliers are required to comply with the requirements in the Ericsson Lists of Banned and Restricted Substances, which include an obligation to provide information on 3TG upon request and to have appropriate due diligence processes in place to identify the source and chain of custody of 3TG. Our first tier suppliers are similarly reliant upon information provided by their suppliers. Certain of Ericsson’s large hardware suppliers are also SEC registrants and subject to the Rule.

The identification of first tier suppliers of Ericsson that are in scope under the Rule involves different regions, business units and production sites that are part of Ericsson’s global organization. The sourcing organization has an important role in leading and coordinating the work. Suppliers identified to be in scope are first tier suppliers of Ericsson supplying (i) products containing 3TG that Ericsson contracts to manufacture, (ii) materials, parts, components and other products, containing 3TG, that are incorporated into or included in a product manufactured by Ericsson, or (iii) materials used in the production of Ericsson’s products where 3TG becomes part of the final product. Suppliers of products that Ericsson does not contract to manufacture (i.e. products purchased “off-the shelf”) and not included in, or incorporated into an Ericsson product have been considered out of scope for purposes of the Rule. In our annual inquiry we survey first tier suppliers identified to be in scope and analyze and follow up responses as we deem appropriate. Accordingly, during 2015, Ericsson conducted a reasonable country of origin inquiry to determine the origin of the 3TG in Ericsson’s products. The inquiry did not provide sufficient information to determine with specificity the countries of origin of the 3TG in our products or whether they are from recycled and scrap sources.

Ericsson believes that industry cooperation is one of the best ways to address issues related to 3TG in an efficient and transparent way. Ericsson is engaged and actively cooperates with other major manufacturers and participates in 3TG industry initiatives such as the Conflict Free Sourcing Initiative (CFSI) driven by the Global e-Sustainability Initiative (GeSI) and the Electronic Industry Citizenship Coalition (EICC). We do not typically have a direct relationship with 3TG smelters and refiners and we do not perform or direct audits of these entities within our supply chain. In accordance with the OECD Due Diligence Guidance for Responsible Supply Chains of Minerals from Conflict-Affected and High-Risk Areas and the related Supplements for gold and for tin, tantalum and tungsten (the “OECD Guidance”), we support the CFSI’s audit of smelters’ and refiners’ due diligence activities. In our internal processes we use data obtained through our membership in the CFSI, using CFSI’s Reasonable Country of Origin Inquiry Report for Ericsson (CFSI member LMEE).

3. Due Diligence

Ericsson has processes in place for the purpose of exercising due diligence in the supply chain. The design of the due diligences processes conforms substantially to the OECD Guidance as it relates to “downstream” purchasers of products and components. The due diligence processes have been described below under sub-headings corresponding to the five-step framework of the OECD Guidance.

3.1 Maintain strong management systems

The first step relates to strong management systems. As mentioned above, Ericsson has established a program team for reporting on 3TG and compliance with the Rule. This team is responsible for the 3TG implementation work and senior management is regularly informed about matters relating to 3TG. Controls include, but are not limited to, our Code of Business Ethics and Code of Conduct, which outlines expected behaviors for all Ericsson’s employees and suppliers, and the Ericsson Lists of Banned and Restricted Substances, which include supplier requirements relating to 3TG as described above and are integrated in our template supplier agreements.

With respect to 2015, identification of smelters/refiners in our supply chain was made via the use of due diligence tools created by the CFSI, including the EICC/GeSI Conflict Minerals Reporting Template 4.01 (the “Template”). The Template is a supply chain survey designed to identify the smelters and refiners that process the necessary 3TG contained in our supply chain. The information from our suppliers, in combination with information from the CFSI, helps Ericsson to obtain information on the origin of minerals processed. Data obtained was reviewed and analyzed by Ericsson and retained in Ericsson’s document handling system.

We have grievance mechanisms whereby employees and suppliers can report violations of Ericsson’s policies. We also have a whistleblower tool, managed by an external service provider, for the reporting of alleged violations of laws or the Code of Business Ethics that (i) are conducted by Group or local management, and (ii) relate to corruption, questionable accounting or auditing matters or otherwise seriously affect vital interests of the Group or personal health and safety. Violations reported through the whistleblower tool are handled by Ericsson’s Group Compliance Forum, consisting of representatives from Ericsson’s internal audit function, Group Function Legal Affairs, Group Security, and Group Function Human Resources. Information regarding incidents is reported to the Audit Committee. Reports include measures taken, details of the responsible Group function and the status of any investigation.

3.2 Identify and assess risk in the supply chain

In the second step, risks are identified and assessed. For downstream companies such as Ericsson, the focus should be on the pinch point in the supply chain, which is the smelters and refiners. Downstream companies should, with help from their first tier suppliers, identify smelters and refiners in the supply chain.

In Ericsson’s 2015 survey, first tier suppliers identified to be in scope were requested by Ericsson to provide information on 3TG in their supply chain through completing and submitting the Template. Suppliers were provided with support material, including the generally available instructions and training illustrating the use of the tool that is available on CFSI’s website. To ensure sufficient quality, a high response rate and to minimize risks, the responses received were continuously analyzed and followed up as deemed appropriate. The data obtained was saved, reviewed and analyzed by Ericsson. Supplier responses were reviewed against criteria developed to determine which responses required further engagement with our suppliers. These criteria were developed to identify whether the responses from suppliers were incomplete and where further inquiries were necessary. Suppliers were contacted via email or telephone to improve the quality of responses and increase the response rate. We have had a risk based approach and focused on suppliers with incomplete data since they are typically deemed to constitute a higher risk. As part of the risk mitigation efforts additional efforts were mainly targeted at suppliers that are awarded large parts of our business on an ongoing basis.

3.3 Design and implement a strategy to respond to identified risks

Under the third step companies are encouraged to design and implement a strategy to respond to identified risks. Due diligence within Ericsson is an on-going process, both proactive and reactive, where suppliers are continuously evaluated against Ericsson’s requirements to ensure that these requirements are systematically incorporated in the suppliers’ processes. Ericsson publishes additional information about its commitment to responsible sourcing and engagement with its suppliers and other stakeholders in its Sustainability and Corporate Responsibility Report, which includes an annual summary of Ericsson’s performance in the areas of responsible business; energy, environment and climate change; and communication for all.

We continuously work to improve our due diligence processes with respect to 3TG, mainly focusing on the quality of data received from suppliers surveyed. This includes communication and collaboration with suppliers for purposes of improving the response rate of surveyed suppliers and improving the reliability and quality of the information provided. As described above, we have a risk based approach when reviewing and following up supplier responses and we generally prioritize to follow up suppliers that we frequently purchase from.

3.4 Independent third party audit of smelters’/refiners’ due diligence practices

The fourth step focuses on audits of smelters’/refiners’ due diligence practices. Through our membership and participation within the CFSI, we support the conflict free smelter program as well as the development and implementation of due diligence practices and tools such as the Template. Due to the structure of our supply chain, there are generally many tiers of suppliers between Ericsson and the smelters and refiners in the supply chain. Therefore, we have, as mentioned in section 2.2, not performed our own audits of smelters or refiners.

We analyze the received data on smelters and refiners and compare such information against the list of facilities that have received a “conflict free” designation from the Conflict Free Smelter Program (“CFSP”). Via the CFSI we also can access information on country of origin and due diligence information on the 3TG sourced by such facilities.

3.5 Report annually on supply chain due diligence

The fifth step encourages reporting on supply chain due diligence. We have a public statement on 3TG on our external website and strive to continuously increase 3TG awareness within Ericsson and among suppliers. Ericsson’s public statement on the sourcing of 3TG is available on our website: http://www.ericsson.com/res/thecompany/docs/corporate-responsibility/statement-on-sourcing-of-conflictminerals.pdf

In accordance with the Rule, we make our Conflict Minerals Report available on our website: http://www.ericsson.com/thecompany/investors/financial-reports.

4. 2015 due diligence results For the calendar year 2015, our inquiries were made on the supplier level and the supplier responses received provided data at a company or divisional level and not isolated to products and components provided to us. Inquiries also include responses where suppliers were unable to specify the smelters or refiners used for components supplied to us and certain responses included names of smelters or refiners that we believe may have been misidentified as smelters and refiners or smelters or refiners that we believe are not operational. Such names have not been listed among identified smelters and refiners. The response rate among suppliers surveyed in 2015 was 81%. A higher response rate was achieved in the group of suppliers that are awarded a larger part of our business. Among the supplier responses, we identified approximately 292 smelters as potential sources of 3TG that were reported to be in the supply chain at some point during the 2015 calendar year. The following table presents, by mineral, the percentages of identified smelters or refiners verified as conflict-free or self-declared in accordance with the TI-CMC Category A members Self-Declared Sourcing Information. Metal Identified smelters or refiners verified as CFSP conflict-free, or self-declared in accordance with the TI-CMC for tungsten1 . Number of identified smelters for each metal Tantalum 100% 46 Tungsten 84% 43 Tin 75% 77 Gold 63% 126 We are unable to validate that any of the identified smelters or refiners are actually in our supply chain. However, based on the information provided by Ericsson’s suppliers during the 2015 calendar year, Ericsson believes that the facilities that may have been used to process the 3TG in Ericsson’s products include the smelters and refiners listed in Annex 1 below. Ericsson does not have sufficient information to determine with specificity the countries of origin of the 3TG in our products or whether they are from recycled and scrap sources. We support the refinement and expansion of the list of participating smelters through our membership in the CFSI program. Through our participation in CFSI, and requesting our first tier suppliers to complete the Template, we have determined that seeking information about 3TG smelters and refiners in our supply chain represents the most reasonable effort we can make to determine the mines or locations of origin of the 3TG in our supply chain. Although, our inquiries are made on the supplier level, targeted activities have been initiated to try to obtain information on the product level. We hope that such efforts may help improve the number of conflict free smelters over time. During 2016, we anticipate to continue to review and evolve our due diligence processes with respect to 3TG. As part of these efforts, we expect to extend the training for suppliers and developing the collaboration with suppliers for purposes of further improving the supplier response rate and the reliability and quality of information provided. Our engagement in the CFSI program is anticipated to continue.

5. Forward Looking Statements Certain of the matters discussed in this report, including in particular our due diligence processes with respect to 3TG, include forward-looking statements. Readers of this document are cautioned that our forward-looking statements are not guarantees of our future actions, which may differ materially from the expectations expressed in the forward-looking statements. We expressly disclaim a duty to provide updates to these forward-looking statements after the date of this filing to reflect events or changes in circumstances or changes in expectations or the occurrence of anticipated events. 3 The CFSI generally does not specify individual countries of origin of the conflict minerals processed by compliant smelters and refiners and thus we were not able to determine with certainty the specific countries of origin of the conflict minerals processed by the listed compliant smelters and refiners. In addition, for some of the listed compliant smelters and refiners, origin information is not disclosed. The list is based on the country of origin data provided to the CFSI for compliant smelters only.

Sanmina Corporation - Conflict Minerals Report For The Year Ended December 31, 2015

Sanmina Corporation Conflict Minerals Report For The Year Ended December 31, 2015

I. Company Background

Sanmina Corporation (“Sanmina”, the “Company”, “we”, “us”, or “our”) is a leading global provider of integrated manufacturing solutions, components, products and repair, logistics and after-market services. We provide these comprehensive offerings primarily to original equipment manufacturers, or OEMs, in the following industries: communications networks, storage, industrial, defense and aerospace, medical, energy and industries that include embedded computing technologies, such as point-of-sale devices, casino gaming and automotive.

II. Performance of Due Diligence on the Source and Chain of Custody of Conflict Minerals

We believe that substantially all of the electronic and electro-mechanical products we manufactured for our customers during 2015 include components that contain Conflict Minerals(1) necessary to the functionality of such products. As a result and as required by Rule 13p-1 under the Securities Exchange Act of 1934 (the “Rule”), we surveyed and performed a reasonable country of origin inquiry (“RCOI”) on a subset of our supplier group that we determined provided components containing Conflict Minerals. The purpose of the RCOI was to determine whether or not the source of such Conflict Minerals was one of the Covered Countries(2) or whether such Conflict Minerals came from recycled or scrap sources.

Based upon our RCOI, we know or have reason to believe that some components used in some of our customers’ products manufactured during 2015 contained Conflict Minerals originating from the Covered Countries and further that such Conflict Minerals were not sourced solely from recycled or scrap sources. As a result and as also required by the Rule, we undertook further due diligence on a smelter group identified to us by our suppliers as providing Conflict Minerals that originated in the Covered Countries. The purpose of this further due diligence was to attempt to ascertain whether the sourcing of Conflict Minerals from these smelters or suspected smelters benefitted armed groups in the Covered Countries. We performed this due diligence in conformity with the Organization for Economic Co-operation and Development Due Diligence Guidelines for Responsible Supply Chains of Minerals from High Risk Areas (the “OECD Guidance”) and related supplements.

III. Design of Due Diligence Process and Alignment to the OECD Guidance

A. Establishment of Strong Internal Company Management Systems

x A Conflict Minerals working group led by our global supply chain organization has established and implemented our Conflict Minerals Policy and regularly reports on its activities to company senior management;
x We use the Conflict Minerals Reporting Template (the “Template”)(3) developed by the Electronic Industry Citizenship Coalition (the “EICC”) to collect Conflict Minerals information from our suppliers;
x We include in our supplier requirements documents and contracts provisions requiring suppliers to recognize our policy and to participate in our RCOI and due diligence activities;
x We maintain records of our RCOI and due diligence activities consistent with our record retention policy; and
x We provide a grievance reporting system that allows reporting of concerns, including those related to Conflict Minerals

B. Identification and assessment of risks in the supply chain

x We employ procedures to identify suppliers which may potentially supply components containing Conflict Minerals;
x We ask these suppliers to report their Conflict Minerals sourcing activities using the Template in an attempt to identify the smelters and refiners involved and the country of origin of such Conflict Minerals;

(1) Conflict Minerals are defined as tin, tantalum, tungsten and gold.
(2) Under the Rule, the Covered Countries are defined as the following countries: Angola, Burundi, Central African Republic, the Republic of the Congo, Rwanda, South Sudan, Tanzania, Uganda and Zambia.
(3) The Template was developed to facilitate disclosure and communication of information regarding smelters that provide material to a company’s supply chain.

x We follow up with suppliers who do not respond by the deadline, requesting their responses;
x We compare smelters and refiners identified by our suppliers against the EICC/GeSI Conflict Free Smelter List (the “Conflict Free Smelter List”)(4) and other third party audit programs; and
x We document country of origin information using various third party sources, such as the smelter database maintained by the EICC/Conflict Free Sourcing Initiative.

C. Design and implementation of a strategy to respond to such risks as they are identified

x We contact direct suppliers to obtain additional information regarding smelters or refiners who may pose a risk of supplying Conflict Minerals that directly or indirectly benefit armed conflict in the Covered Countries;
x We work to improve our suppliers’ understanding of the Rule and to improve their response rate to our queries and the quality of their responses, primarily through enhancing our supplier requirements documents and through follow-up conversations during our due diligence process;
x We utilize the Conflict Free Smelter List to identify smelters not participating in the Conflict Free Smelter Program operated by the Conflict Free Sourcing Initiative (“CFSI”) and request, through the suppliers that have identified them, that they participate in such program;
x We bring to the attention of our customers the discovery of information that leads us to conclude that the sourcing of components or materials for our engagement with them if required by them benefits armed groups in the Covered Countries;
x When the decision of components sourcing is within our control, we will take steps over time to utilize suppliers who do not source their Conflict Minerals from the Covered Countries or who participate in conflict-free smelter programs; and
x Our Conflict Minerals working group reports the results of our due diligence efforts to Company management and the Audit Committee of our Board of Directors.

D. Support industry organization to carry out third party audits of smelters and refiners facilities and processes

x We maintain membership in the EICC, which develops and implements audit procedures for refiners and smelters

E. Report on Due Diligence measures
x We publicly disclose our Conflict Minerals policy on our corporate web site at http://www.sanmina.com/companyprofile/conflict-minerals-policy/index.php; and
x We publicly report on our due diligence efforts on an annual basis through this Conflict Minerals Report available at http://www.sanmina.com/company-profile/conflict-minerals-information/index.php.

IV. Description of Due Diligence Measures performed
A. We conducted a survey of suppliers whose products may include Conflict Minerals using the Template.
B. We evaluated suppliers’ responses to the Template questions, identified inconsistencies and answers requiring follow-up, and followed up with those suppliers to clarify their responses.
C. We retained an independent due diligence firm to provide us with further information about the smelters identified to us through our due diligence process.
D. We confirmed whether the smelters identified by our suppliers through the Template were included on the Conflict Free Smelter List.
E. For smelters not included on the Conflict Free Smelter List, we requested, through the suppliers that identified them, that they participate in such program.

(4) The Conflict Free Smelter List is generally understood to contain the names of smelters from whom procurement of Conflict Minerals does not benefit armed groups in the Covered Countries. However, we do not express an opinion as to whether or not the sourcing of Conflict Minerals from these smelters actually does not benefit armed groups in the Covered Countries.

B. Country of Origin of Necessary Conflict Minerals Contained in Sanmina Products, If Known

As a result of our efforts to determine the mine or location of origin of Conflict Minerals used in our products, as described in Section C below, we believe the countries of origin of the Conflict Minerals used in our products include: Angola, Argentina, Australia, Austria, Belarus, Belgium, Bolivia, Brazil, Burundi, Cambodia, Canada, Central African Republic, Chile, China, Colombia, Côte D’Ivoire, Czech Republic, Democratic Republic of Congo, Djibouti, Ecuador, Egypt, Estonia, Ethiopia, France, Germany, Guyana, Hong Kong, Hungary, India, Indonesia, Ireland, Israel, Japan, Kazakhstan, Kenya, Laos, Luxembourg, Madagascar, Malaysia, Mexico, Mongolia, Mozambique, Myanmar, Namibia, Netherlands, New Guinea, Nigeria, Peru, Philippines, Poland, Portugal, Republic of Congo, Russia, Rwanda, Sierra Leone, Singapore, Slovakia, South Africa, South Korea, South Sudan, Spain, Suriname, Switzerland, Taiwan, Tanzania, Thailand, Uganda, United Kingdom, United States of America, Vietnam, Zambia

C. Efforts to Determine Mine or Location of Origin

Our efforts to determine mine or location of origin of Conflict Minerals used in our products consisted primarily of our participation in the EICC and our review of information reported by our suppliers. As described above, we obtain smelter information, including when reported, mine or location of origin, from our suppliers using the Template. In addition, we sought additional information on mines and locations of origin using the CFSI’s Smelter Database. Finally, we retained an external consultant to review any additional publicly available information regarding mines and locations of origin for the smelters reported to us by our suppliers.

D. Steps to Improve Our Due Diligence in 2016

x In situations in which we control the sourcing decision, we will encourage suppliers which have reported use of smelters not listed in the Conflict Free Smelter List in the past to cause the such smelters to participate in the Conflict Free Smelter Program or similar programs.
x We will use information gathered for our 2015 Conflict Minerals reporting activities to refine the list of suppliers to which we send the Template and to focus more outreach efforts on suppliers who have in the past provided information requiring further validation.
